The Second Step is to complete and submit the Medical History form and Patient Registration form. You may submit the completed Registration and Medical History Form by fax to 703-995-4928 or mail it to MicroSort, 3015 Williams Dr., Suite 101, Fairfax, VA 22031. If applying for genetic issues, please also send medical documentation of genetic disorder. The forms will be reviewed to verify qualification into the trial. You will be contacted by phone informing you as to whether you have been approved or declined for the trial. If approved a consult can be scheduled for you.
